What Documents Will You Need?

It depends. If the applicant is charged and prosecuted for a Felony or Misdemeanor, the applicant will need to bring all court documents related to the charge in which he/she was prosecuted. However, if the applicant was arrested and charged but not prosecuted, he/she will need to bring in Court Dismissal documents for a dismissed charge.

What Names Do We Use? (Dl, Ids, Ss)

Applicants must list the name listed on their SS card. If the applicant is married but hasn’t legally changed last name, he/she will still need to list the name as it appears on the SS card.
